Personalised care plans and ongoing support for people living with chronic health conditions.
Living with a chronic health condition can involve regular appointments, medications, tests and sometimes care from several different health professionals.
At North Brighton Medical, our GPs and practice nurses work together to help patients manage long-term health conditions, prevent complications and maintain their quality of life.
We offer bulk-billed GP Chronic Condition Management Plans and regular care plan reviews for eligible Medicare patients, helping make ongoing healthcare more accessible.
What Is a GP Chronic Condition Management Plan?
A GP Chronic Condition Management Plan (GPCCMP) is a personalised healthcare plan developed with your GP to help manage one or more ongoing medical conditions.
Your plan may include:
A review of your medical history, current health and medications.
Identification of your individual health needs and goals.
Advice on managing your condition and reducing the risk of complications.
Coordination of care between your GP, practice nurse, specialists and allied health professionals.
Referrals to other healthcare providers where appropriate.
Regular reviews to monitor your progress and update your treatment.
The aim is to provide coordinated, ongoing care rather than simply responding to health problems as they arise.
Who Is Eligible for a Chronic Condition Management Plan?
You may be eligible if you have a medical condition that has lasted, or is expected to last, at least six months, or a terminal medical condition.
Examples of conditions that may benefit from a care plan include:
Diabetes
Heart disease and high blood pressure
Asthma and chronic obstructive pulmonary disease (COPD)
Arthritis and other chronic musculoskeletal conditions
Osteoporosis
Chronic kidney disease
Other ongoing medical conditions requiring coordinated care
Your GP will assess whether a care plan is appropriate for your individual circumstances.
Generally, care plans are prepared by your regular GP or another GP at your usual medical practice. If you're registered with MyMedicare, your care plan must be provided through your registered practice.
You can change your registration to North Brighton Medical if you meet the MyMedicare eligibility requirements. Complete our MyMedicare registration form here.
Regular Reviews to Keep Your Health on Track
Managing a chronic condition is an ongoing process, and your healthcare needs may change over time.
Your GP Chronic Condition Management Plan can generally be reviewed every three months, allowing your doctor to monitor your progress, review medications and test results, and adjust your treatment where necessary.
These appointments are a valuable opportunity to discuss how you're managing your condition, address new concerns relevant to your care plan and plan the next steps in your treatment.
Eligible care plan reviews are bulk billed at North Brighton Medical.
Our practice nurses may also assist with health assessments, monitoring and education as part of your ongoing care.
Access to Medicare-Funded Allied Health Services
If you have a chronic health condition, you may benefit from treatment provided by other health professionals alongside your GP.
Patients with an eligible GP Chronic Condition Management Plan may be able to access Medicare rebates for up to five individual allied health services per calendar year, subject to Medicare requirements.
Depending on your needs, this may include services from:
Physiotherapists
Podiatrists
Dietitians
Exercise physiologists
Other eligible allied health professionals
Your GP can discuss whether allied health referrals would benefit you and help coordinate your care.
Please note that Medicare rebates may not cover the full cost of allied health appointments, and out-of-pocket fees may apply.
Are Chronic Disease Management Appointments Bulk Billed?
Yes. North Brighton Medical bulk bills eligible GP Chronic Condition Management Plans and care plan reviews for patients with a valid Medicare card.
This includes regular reviews, generally available every three months, where Medicare eligibility requirements are met.
Standard consultation fees may apply to other appointments, including consultations for unrelated medical concerns or services that don't meet the Medicare requirements for a care plan or review.
If you're unsure whether you're eligible, our reception team can help arrange an appropriate appointment.
