All new GP patients of North Brighton Medical must complete a New Patient Registration Form, even if they have seen one of our GPs before at another clinic.

You can:

do this electronically - your SMS and/or email appointment reminder will have a link to an electronic form, or you can click on the button below; or
arrive 10 minutes early to complete a paper form in our reception area - due to COVID-19, we are trying to avoid this option as much as possible so that patients do not need to touch our pens and clipboards and to minimise the amount of time people spend in the clinic.
