We offer two types of Health Assessment: one for patients aged 75 and over, and one for patients aged 45-49.

Coming soon: we will soon be offering a Health Heart Check for patients aged 45 and over (or 30 and over if you're of Aboriginal or Torres Strait Islander descent).

Read below for more details of each type of Health Assessment. Remember that prevention really is better than cure!

aged 75 or over?

If you are aged 75 or over, you may be eligible for a FREE comprehensive health assessment with our nurse Sharon Cimino and your usual GP. Most patients are eligible for a health assessment once every 12 months.

A 75+ Health Assessment involves our nurse Sharon coming to your house to interview you, and check your blood pressure and other health metrics to determine if you have any changing health needs.

You will then have an appointment with your usual GP, who can then discuss your individual health needs and offer advice on how to improve your overall health, prevent illness and live longer and with better quality.

The visit from Sharon takes approximately 45 mins, and you then see the GP for around 30 mins. It is fully Medicare funded.

aged 45 to 49?

If you are aged 45-49, you may be eligible for a one-off FREE comprehensive health assessment with one of our nurses (Sharon Cimino or Lisa Sanders) and your usual GP.

A 45-49 Health Assessment involves our nurse Sharon interviewing you, reviewing your recent medical and family history, and checking your blood pressure and other health metrics to determine your risk of developing a chronic condition such as heart disease, diabetes or cancer.

You will then have an appointment with your usual GP, who can then discuss your individual health needs, consider additional medical tests and offer advice on how to lower your risk factors so that you can improve your overall health, prevent illness and live longer and with better quality.

The full assessment takes approximately one hour, and is fully Medicare funded.
